Qualifications and Eligibility Criteria 

• A bachelor’s degree in public health, business administration, health care management, or a similar discipline.
• 8+ years of program management experience, with at least 3 of those years spent in a senior position.
• A track record of successfully overseeing intricate, multi-stakeholder programs.
• Strong familiarity with project management techniques like Scrum, Agile, or PMI.
• Outstanding interpersonal, leadership, and communication abilities.
• Depending on the year, relevant experience may be used in place of the educational requirement.

Role and Responsibilities:

• To guarantee that the program complies with DHCS Medi-Cal and CalAIM ECM and CS rules, policies, and ILS health plan contracts, lead the creation, improvement, and implementation of program protocols, workflows, training, and quality improvement projects in coordination with operational leaders.
• Work together with stakeholders and executive leadership to establish program objectives, deliverables, schedules, and resource needs for the implementation of community support and care coordination/case management initiatives.
• Guarantees the standardization and optimization of care models’ workflows as they are expanded and grown.
• Creates playbooks for community support and care management programs that are prepared for expansion and scaling, tracks program performance indicators, recognizes risks and problems, and puts mitigation plans into place to guarantee effective implementation.
• To coordinate efforts and address dependencies, help cross-functional teams communicate and coordinate.
• Oversee program financial reporting, forecasts, and budgets to maintain transparency and economic discipline.
• Continuously assesses cutting-edge, empirically supported research on community support initiatives and care management.
• Analyze program results and apply lessons learned to future planning to spearhead efforts for continual improvement.
